Liability Release Form Template
As part of its research mission, the University of Hawaii at Manoa (UHM) encourages collaborative endeavors between UHM personnel and visiting non-UHM personnel, including students.
Such endeavors may involve potentially hazardous materials, equipment, operations or conditions. Each Dean, Director, and Department Chair should remind faculty hosting visitors to require each visitor to sign a release of liability form prior to their engaging in research activities at UHM facilities or with UHM collaborators.
Please use the liability Release Form template which you can download and should modify for your specific laboratory.
Submit all liability Release Forms to the UH General Counsel’s office for approval prior to obtaining signatures from visiting personnel and students. Approval is required before visiting scholars and students engage in collaborative research activities.
